Brandon is a native of Houston, Texas, and brings his passion for human movement and wide-ranging expertise to his practice, combining over 15 years of experience as a personal trainer and Fascia Stretch Therapy practitioner​.

Brandon was first certified as a personal trainer through the National Academy of Sports Medicine (NASM) in 2009. He followed that up in 2010 with advanced trainings as a Corrective Exercise Specialist and Sport Performance Specialist, also through NASM.

In addition to sport and exercise training he’s also trained and certified by Precision Nutrition as a Level 2 nutrition coach with a specialization in Athlete Nutrition. Wanting to deepen his expertise and learn a more hands on approach to help people get out of pain and perform better in sport and everyday life, he was certified by the Stretch to Win Institute in October 2010 as a Fascia Stretch Therapist.

He has since achieved their highest level of credentialing by becoming a Level 3 Fascia Stretch Specialist. Throughout his 15 year career he has worked with professional athletes from almost every major sport including NFL, NBA, MLB and PGA.

We provide the required documentation—including a medical necessity form and itemized receipt—to help you submit for reimbursement through your HSA or FSA provider. Q: Can I really get reimbursed for my gym membership?
A: Yes—if your doctor deems it medically necessary. Many HSA and FSA plans allow gym memberships to be reimbursed when prescribed as part of a treatment for conditions like obesity, hypertension, diabetes, or anxiety.

Q: What documents do I need to submit?
A: You’ll need two things:

  1. A completed Medical Necessity Form signed by your licensed healthcare provider.
  2. An itemized receipt from OAK Fitness for your qualifying membership.
 

Q: How do I submit my paperwork?
A: Download and Submit the signed form and your receipt directly to your HSA or FSA plan administrator. Most providers offer online submission portals or mobile apps for uploading documents. Download the form by clicking this link.

Q: Who is my HSA/FSA provider?
A: Your provider might be a company like Optum Bank, HSA Bank, HealthEquity, WageWorks, or Fidelity. You can typically find this information on your benefits portal or by asking your employer.

Q: Does OAK Fitness guarantee reimbursement?
A: No. Approval is entirely up to your HSA/FSA provider’s policies. We provide the necessary forms, but we recommend checking with your provider before submitting.
